Graduation Forms

Application for Graduation

The Application for Graduation is available for all potential graduates from all programs of the University.  This forms should be completed and returned to the Registrar's Office with 1-3 semesters of the term a student plans to graduate.  The Application for Graduation should be submitted even if there are no plans to participate in spring commencement ceremonies.

Diploma Replacement Form

The Diploma Replacement form is available for any graduate of Bethany that may need a replacement or second copy of their diploma.  Replacement Diplomas cost $30.  Mailing and faxing information is available on the form.

Registration Forms

Change of Program

The Change of Program form is used when a current student desires to change from one program to another.  The undergraduate programs include CPAC, Traditional, eBethany, and APPL.  The graduate programs include Master in Teaching and Master in Clinical Psychology.

Change of Schedule

A Change of Schedule form is used to change a course schedule after a current student has initial registered for courses in a given semester.

Petition to Exceed 18 Units

A Petition to Exceed 18 units is submitted if a current student desires to take more than a Full-Load plus 1 unit (18 units) in a semester.  This petition is approved based on both Academic Advisor Approval and current G.P.A. standing

Special Course Approval

A Special Course Approval is used when a current student desires to take a course that is not regularly offered during the current semester or by Independent Study.  Special courses must have the approval of both the instructor requested to teach the course and the Academic Advisor.

Degree Evaluation and Transfer Forms

Declaration of Major

A Declaration of Major is a one time only form used when a prospective or current student initially declares their intent of major.

Change of Major

A Change of Major is used each time a change is made to a declared major or minor.

Program Modifications

Program Modifications are used to modify current degree course requirements within a major for individual students.  Program Modifications require the processing and approval of the student's Academic Advisor.  They are available in two forms: one for general education requirements and one for major requirements.

Information Release

Information Release

Under FERPA (Family Education Rights to Privacy Act), Bethany can only release directory information regarding their current students to outside parties.  If a student desires someone else (i.e. parent, spouse) to access their student records they must submit an Information Release form.

Non-Disclosure Request

Under FERPA (Family Education Rights to Privacy Act) Bethany can release directory information to outside parties.  If a current student desires that their directory information not be available for release they must submit the Non-Disclosure Request.  The Non-Disclosure Request should be submitted each semester a student is in attendance for continual validation.
